which may be of assistance to the public. The forms listed below may be downloaded and then
printed. You may be asked by a Corrections Officer to print a form from this page,
fill in necessary information, mail it, or drop off at DCDF.

Huber Privilege Information Sheet
The DCDF
Huber Privilege Information
sheet contains important information for those required to serve a
jail sentence with Huber privilege. It contains instructions about
what information is needed prior to being allowed to work or leave
jail for court ordered child care, and other valuable information.
GPS
Application
GPS (also known as electronic monitoring or the
"bracelet"), allows an inmate to reside in his/her home while
serving a sentence. The inmate wears an ankle bracelet that monitors
his/her location. Anytime the inmate is outside of the home must be
previously approved by a Programs Officer. This application may be
completed after sentencing and dropped off at the main lobby or
mailed to:

Huber
Privilege Transfer Request
If the judge grants transfer
privileges and the judgment states transfer privileges to a specific
county to be served on electronic monitoring, a Huber Privilege
Transfer Request may be completed and dropped off at the main lobby
or mailed to:

This form is only to be used when sentenced in Dodge
County. If someone is sentenced in another county and wants to
transfer to Dodge County, he/she must contact the sentencing county
and request a transfer from that county. Transfers to Dodge County
cannot be initiated by the person who was sentenced.
GPS
Work Schedule
When an inmate is serving time on GPS, the GPS
Work Schedule must be completed and turned in at the main
lobby.
Inmate
Visitation Form
If you wish to be added to an Inmate’s visiting list,
complete this form. Send this form to the inmate so that they can
submit it. You should call before you come to visit to verify that
you have been approved. All information must be accurate.
Midweek/Workender Application
This application is to be filled out when directed to do so by a
DCDF Programs Officer responsible for the Midweek/Workender program.
This application will be printed, filled out, and dropped off or
